    A beneficiary typically refers to someone eligible to receive distributions from a trust or a Will. The beneficiary or beneficiaries outlined in your Will are the individuals who you choose to pass on your property or belongings to when you are no longer living.

  • Part F – Guardianship of Minor Children (If Applicable)

    Please Note: The named guardian must consent to have custody of any minor child(ren) upon your death and must apply to a court of competent jurisdiction within (90) days of your death for appointment as the custodian and guardian of property of said children. Your lawyer will discuss this with you.
